    Itamar Medical and its affiliates (“Itamar Medical", “we” or "us") respect the privacy of our users (“User” or “you”) and are committed to protect User's information.  We believe that you have a right to know our practices regarding the information we may collect and use when you visit and/or access the information and/or vouchers and/or services provided through the website at the address  http://www.prevent-heart-attack.com/ or any other Itamar Medical website under the domain Itamar-medical.com (hereinafter, the “Site”).

    Information Collection and Use

    1.INFORMATION ABOUT YOU.  When you register with us on the Site to create an account ("Account") and become a registered user ("User"), you will need to provide us with certain personally identifiable information to complete the registration, including information that can be used to contact or identify you ("Personal Information").  Personal Information that we may ask you to provide includes, for instance, your name, email address, credit card or other billing information.  We use this Personal Information mainly to provide the Services, administer your inquiries and process payments.

    HEALTH INFORMATION.  In connection with your use of the Service, Itamar may also collect personal health information provided by you or generated through your use of the Site.  By using the Site, you explicitly authorize Itamar to collect personal health information, and you represent that you are authorized to disclose any and all information you provide.  YOU FURTHER ACKKNOWLEDGE AND AGREE THAT ANY PERSONAL HEALTH INFORMATION PROVIDED BY YOU OR GENERATED THROUGH YOUR USE OF THE SITE IS NOT CONSIDERED PROTECTED HEALTH INFORMATION (PHI) UNDER THE HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 (HIPAA), AND THAT SUCH PERSONAL HEALTH INFORMATION THAT YOU PROVIDE ON THE SITE SHALL NOT BE CONSIDERED CONFIDENTIAL OR PRIVATE UNDER ANY STATE LAW.  

    OTHER INFORMATION.  We collect information that you provide as part of registration, the administration and personalization of your user experience, including certain demographic information such as your gender, age and zip code, and such information as you may submit from time to time, including applications for vouchers.  We may combine this information with the Personal Information we collect from you, and use it to provide the Services, complete your transactions, and administer your inquiries.  We may also use this information to improve user experiences on our Site, to improve the quality and value of the Services and to analyze and understand how our Service is used.  In addition, we may use it to serve you specifically, for instance to deliver a product to you according to your preferences or restrictions.

    Non-Personal information is collected from you while you access or use the Site, for example, when you connect to the Site or available on the Site, your computer may tell us your IP address.     Log Data.  When you visit the Site or use the App, whether as a Member or a non-registered user, our servers automatically record information that your browser sends whenever you visit a website ("Log Data"). This Log Data may include information such as your computer's Internet Protocol ("IP") address, browser type, operating system or the webpage you were visiting before you came to our Site, pages of our Site that you visit, the time spent on those pages, access times and dates, and other statistics.  We use this information to monitor and analyze use of the Site and the Services and for the Site's technical administration, to increase our Site's functionality and user-friendliness, and to better tailor it to our visitors' needs. For example, we use this information to verify that visitors to the Site meet the criteria required to process their requests.

    Cookies. Like many websites, we use "cookies" to collect information. A cookie is a small data file that we transfer to your computer's hard disk for record-keeping purposes.     Clear Gifs (Web Beacons/Web Bugs). We or the third parties we engage to track and analyze Website data may employ clear gifs (a.k.a. Web Beacons/Web Bugs) to help us better manage content on the Website. Clear gifs are tiny graphics with a unique identifier, similar in function to cookies, and are used to track the online movements of Web users. In contrast to cookies, which are stored on your computer's hard drive, clear gifs are embedded invisibly on Web pages.

    NOTWITHSTANDING ANYTHING ELSE IN THIS PRIVACY POLICY AND/OR THE TERMS OF USE, WE MAY ALSO COLLECT AND USE AGGREGATE, DE-IDENTIFIED, OR OTHER ANONYMOUS INFORMATION ABOUT USERS FOR MARKETING, ADVERTISING, RESEARCH OR OTHER PURPOSES.
    1.Purposes of the Collection of Information?

    Non-Personal Information is collected in order to:    
    Enhance the User's experience on the Site (e.g. by determining the screen resolutions).
    Learn about the preferences of Users and general trends on the Site (e.g. understand which features are more popular than others).
    Produce aggregated data such as measurements and statistics regarding the overall usage of the Site.

    Personal Information is collected in order to:    
    Allow registered Users to request information and/or education on-line and/or through the chat function, when available.
    Allow us to contact Users who submitted a request through the Site.

    3. Opt-out
    At any time you may opt out of our mailing lists, by sending us an email to: info@itamar-medical.com. In any case, we will not share your Personal Information with any advertisers, without your explicit consent. It may take up to 10 days for your opt-out request to take effect.


    4. Deletion of Personal Information
    If for any reason you wish to delete your Personal Information from our database, please send us an email to: info@itamar-medical.com, and we will make reasonable efforts to delete any such Personal Information pursuant to any applicable privacy laws. However, note that we may not be able to provide you in the future with the all the services available on the Site.


    5.Security
    We take maintain the security of the Site and your information and in preventing unauthorized access to it through industry standard technologies and internal procedures. However, we do not guarantee that unauthorized access will never occur.


    6.Third Party Websites
    While using the Site you may encounter links to third party websites, such as of advertisements or related goods and services. Please be advised that such third party websites are independent from the Site, and we assume no responsibility or liability whatsoever with regard to privacy matters or any other legal matter with respect to such third party websites.  We encourage you to carefully read the privacy policies and the terms of use or service of such websites.

    The terms of this Privacy Policy will govern the use of the Site and any information collected therein.  Itamar Medical reserves the right to change this policy at any time, so please re-visit this page frequently.  In case of any material change, we will make reasonable efforts to post a clear notice on the Site.
    Changes to this Privacy Policy are effective as of the stated “Last Revised” date and your continued use of the Site after the Last Revised date will constitute acceptance of, and agreement to be bound by, those changes.
